Although every effort is made to contact your insurance carrier to determine eligibility, benefit information, and the authorization for services, it is your responsibility to understand your insurance coverage and obtain authorization for services. Not all services are covered in all insurance contracts. If your insurance plan does not cover a service or procedure, you are responsible for payment of these charges.
Please contact your insurance company before your first visit to Rehabilitative Services. Contact your insurance company by calling the customer service number on your insurance card. Your human resource department may also be a source of information for any questions.
Some insurance companies require a prescription from the physician whereas others require a referral and authorization. You may schedule an appointment prior to receiving authorization, referral, or a prescription but the child may not be seen by a therapist until the prescription is received. If a referral or authorization is not obtained before the appointment, you will be financially responsible for the payment.
