Social Work and Behavior Modification Class
The patient will meet with a social worker at the initial clinic visit to assess for potential barriers to enrollment and successful program completion. The social worker may also provide information regarding needed resources such as financial assistance and/or counseling services pertinent to the patient and their family. These services are provided on an individual basis. Once enrolled in the program, patients and parents are expected to attend 80 percent of Behavior Modification Education classes. These classes are held once a week in conjunction with Nutrition Education classes.
